Contract for Care Volunteer Commitment Letter
Dear [volunteer]:
We are pleased to let you know that Franklin Memorial Hospital has accepted your offer to provide volunteer work at the hospital. We ask you to sign the enclosed form that explains the hospital's Contract for Care Program. By signing the form and returning it to us you agree to be a volunteer at the hospital.
Your offer of volunteer work lets the hospital use current funds to provide free or reduced-price care to current and future patients. In other words, while your promise to help FMH with needed volunteer work does not have any direct connection to the hospital’s decision to credit your account, your commitment to support the hospital through volunteer services makes hospital funds, that would otherwise be necessary to obtain services that you are offering to provide, available to fund the hospital’s continuing community service.
We are crediting your account [or the account of someone else on behalf of whom you are volunteering] in the amount of $xx , leaving a zero balance. We are crediting this account in appreciation of your public spirited commitment. In order for us to carry out this program, please fill out and sign the enclosed form and return it to our Volunteer Services Department in the enclosed envelope.
