Franklin Health Access
Franklin Community Health Network offers this program in partnership with HealthReach Community Health Centers and others. Local doctors and providers donate their services at a reduced rate for this program. Franklin Health Access also reimburses a portion of prescription drug costs. People may qualify on a sliding scale up to 250% of the Federal Poverty Guidelines.
You need to have legal residence in Greater Franklin County to apply for this program. Most local health care services are covered and Franklin Health Access also offers help connecting people with other community resources. Enrollees are assigned a personal health advocate and get a free comprehensive, health risk screening.

Who can become a member?
Franklin Health Access welcomes people of all ages that have no health insurance or insurance with a deductible of $5000 or greater and that meet the eligibility criteria.

How will the program help me?
If you do not have a doctor, we will help you choose one that participates in this program.
You will be assigned a Personal Health Advocate who will help you with your health care needs and also refer you to other available community resources.
You will receive services based on a sliding-fee scale, and free self-help information. You are also eligible for a free ScoreHealth screening for cholesterol, blood pressure, and heart risk factors, and for prescription reimbursements of 50% of your cost up to $100 (maximum). We also assist you with applications to patient assistance programs for free medications.
What will my responsibilities be?
You will be responsible to pay your share of the sliding-fee, called your co-pay. Co-pay levels range from 0% (co-pay of $5 per visit) to 80% of the bill, depending on your income. You will need to tell us about any changes in your income or household size. You will be responsible for your own health care. Your doctor and Personal Health Advocate will assist you as needed.
How do I enroll?
Complete an enrollment form and provide proof of income. To obtain a form or for more information please call our office at 779-2772 or 1-888-952-2772. Persons that use a TTY can call 779-2662. We will be happy to assist you in filling out the form.
What doesn't this program do?
