Office Information

We are located in the Androscoggin Valley Medical Arts Center and are open Monday through Friday from 7 a.m. to 5 p.m.

  • We are accepting new patients
  • Same day appointments are often available.
  • If you have any questions, please feel free to give our office a call and someone will be happy to help you.

We offer 24-hour call coverage when the office is closed, including weekends and holidays. Simply call the medical practice and listen to the instructions on how to reach the on call provider. If you do not need immediate help, you can leave a message on our answering machine and we will return your call during office hours.

Our practice has a skilled licensed clinical social worker (LCSW) to help facilitate behavior health needs for our patients.This allows the entire health care team to work collaboratively to meet the needs of our patients

Franklin Health Livermore Falls Family Practic is a level 3 patient-centered medical home, a widely recognized symbol of quality. This recognition demonstrates that the practice provides its patients with the right care at the right time, including: same day appointments as needed, expanded hours, 24/7 access to a provider, and direct delivery of behavioral health services by a Licensed Clinical Social Worker as needed.

Prescription Refills

Please contact us at least 48 hours before you will need to have prescriptions refilled.

Appointment Policy

If you are unable to make your appointment, 24-hour advance notice is appreciated whenever possible. This allows us to open up that time for another patient needing care.


In an extreme emergency, such as loss of consciousness, chest pain, severe breathing problems, or major bleeding, call your emergency service number (usually 911) first. Do not delay emergency care by calling our office or the hospital. Afterward, if necessary, call your physician for further advice and coordination of your emergency care.
